Medical Marijuana Identification Card
The California Department of Public Health’s (CDPH) Medical Marijuana Identification Card Program (MMICP) was specifically established to create a State-authorized medical marijuana identification card, along with a registry database for verification of qualified patients and their primary caregivers. Participation by patients and primary caregivers in this identification card program is voluntary. The MMP Web-based registry allows law enforcement and the public to verify the validity of a qualified patient or primary caregiver’s MMIC as authorization to possess, grow, transport, and/or use medical marijuana within California
You will need to fill out an Application/Renewal Form. You must reside in the California county where the application is submitted. You will need to provide current documentation with your application as follows:
*A copy of your doctor’s recommendation
*Proof of identity.
-This can be a valid California Department of Motor Vehicles (DMV) driver’s license or valid identification (ID) card or other valid government-issued photo ID card.
*Proof of residency which can be:
-Rent or mortgage agreement.
-California DMV motor vehicle registration.
